Transparent Classroom (TC) is your online portal for *medical forms, conference reports, your classroom directory, and photos of your child.

• *Medical Forms: Children are not permitted to administer their own medication, vitamins, cough drops, Chapstick, Aquafer, pain relievers, etc. while at school. In order for staff to administer medication of any kind during the school day, the proper form must be filled out and submitted.
  • Medication Authorization Form: If your child needs to take medication, whether prescription or “over the counter,” during the school day, for a non-life-threatening condition (e.g. eye drops, cough drops, Chapstick, ibuprofen, vitamins, diaper cream, etc.) you will need to fill out the Medication Authorization Form.
  • Medical Action Plan: If your child has a serious diagnosed allergy or other medical condition requiring a medical protocol (for example: allergies which may create a breathing emergency, a need for epi pens, antihistamine, seizures, severe asthma, etc.) you will need to fill out the Medication Authorization Form and submit a Medical Action Plan.
